Number of deaths

What does number of deaths in the episode progression list mean? Based on that the game gives you an A, B or C grade but what what does number of deaths mean? I can't get more than 0/1.

Basically, you'll get a perfect score if you don't die in that episode. So 0/1 means you've died 0 times which is what your scoring for throughout the whole episode. If you do die, it will mess you up in the end making you not get any gold skulls.
