Equipment disappeared.

I'm sorry if this is a double post on my sure, if the other one went through.
I created a Raw hand exerciser IV for one of my superstars, and then it just disappeared, the inventory of the equipment I used to make it disappeared, so I know what was created, but now it's not there, it took me a lot of time and effort to get it, but now it's gone, has anybody else had this problem
